Are you facing a problem that is surfacing again and again? Wondering how to solve the issue permanently? Try the Five Whys Analysis method to identify the root cause of the problem. Simply ask a sequence of five “why” questions to pass through the obvious reasons and arrive at the true reason.
For example, in the above chart the problem was first defined and then a series of “why” questions were asked to reveal the root cause.
Depending on the criticality of the problem, sometimes the root cause identification might take fewer than five whys analysis questions, and other times more than five why questions.
